1. Purchase Nutrients Rx Blood Work above or you can also ask your Health Professional to send you the Nutrients Rx Lab Work using Direct Script.
2. You will get an email to register your Nutrients Rx account. Please create your password for the OHS Health Portal and fill out your personal information (i.e. Name, DOB, Height, and Weight).
3. After you complete Registration, an email will be sent to confirm your email address, and a four digit 2FA (2 factor authentication) code will be sent to your email. Use that four digit code to complete your login to your Health Portal account, then answer the Health Questionnaire.
4. For your blood and lab work:
a. Go to your Portal HEREb. Download the PSC Hold PDF from your Dashboard. This is your blood request form called Patient Service Center (PSC) Hold.c. Print off your PSC Hold PDF.d. Search for a LabCorp Lab near you by clicking “Find a Testing Location.” (If you prefer Quest Diagnostics lab, please contact us to update your PSC Hold)e. On the new page, search for a lab testing location using your ZIP code to find and schedule an appointment.f. On the day of your appointment, fasting and a urine sample is required for the labs. Also, be sure to take your printed PSC Hold PDF.
5. After your blood and lab testing is done, please wait 14-16 business days for us to receive your results from the lab, review them, create your comprehensive Health Report, and Create your Custom Health Pak.
6. When your results are ready, you will receive an email that will let you know that your Health Report is ready for you to download and review from your Nutrients Rx Portal and that you can order your Custom Health Pak.
7. At this point, your Custom Health Pak has been updated to include the exact set of nutrients that your body needs. You will receive an email to purchase your customized supplements - Get set up with automatic shipment so you never have to stress over reordering or running out!
8. Simply take one packet morning and evening every day of the custom pak and work to improve your lifestyle to help your body have the health changes you desire!
9. At six months, we’ll send you a reminder email to update your Questionnaire and retest your blood work. This ensures that your needs are continually being met and that you can see the data on how your health is improving. If changes to your supplement paks are needed from your new results, we’ll adjust automatically to ensure you the best results.
10. After renewing blood work, you can easily compare previous results side-by-side to the new ones so that you can know you are lowering your risk for disease, increasing your energy, and achieving optimal health.

I am highly impressed with Nutrients Rx! First of all, all the bio markers that the labs cover are so much more than “traditional “ blood work up. I am not satisfied at all with the company. You have mixed up orders between my wife and myself. I now have two months supply and she has none. We were also promised free blood work when we originally signed up and now you want to charge us $100. We called to straighten things out. No one knew what to do. Said they would ask someone and call us back. To this date no one has returned our call. I am canceling my account if this matter is not resolved.
